You need to save the form
(file) onto your hard drive first before using the form.
They are in a 'locked' document format and cannot be filled out online.
This is the pop up screen that you should see:

Click on save ......... then this pop up screen is what you will see:

Select the folder that you
want to save this in at the "Save in:" dialog box. As you can see - I
created a folder named "Forms - Computer" in my Referee folder and renamed them
with
_blank at the end. These are the files that I pull up to submit a report.
Open the 'blank' form in MS Word or Excel

Use your "tab" key to go
from field to field and fill out the form - when done - go to
<file><save as>
I created a folder called
"Misconduct Reports" in my referee file to stuff the reports in.
I rename the file into something that makes sense to me so I can go back to
review if
needed - then click <save>. This action renames the file and closes the
original _blank
version - ready to be used on the next report.
I then print out a hard copy to mail into the disciplinary committee.
