Back to Forms

Download Instructions for MS Word or Excel forms (pc only)

You need to save the form (file) onto your hard drive first before using the form.
They are in a 'locked' document format and cannot be filled out online.

This is the pop up screen that you should see:

Click on save ......... then this pop up screen is what you will see:

Select the folder that you want to save this in at the "Save in:" dialog box. As you can see - I
created a folder named "Forms - Computer" in my Referee folder and renamed them with
_blank at the end. These are the files that I pull up to submit a report.

Open the 'blank' form in MS Word or Excel

Use your "tab" key to go from field to field and fill out the form - when done - go to
<file><save as>

I created a folder called "Misconduct Reports" in my referee file to stuff the reports in.
I rename the file into something that makes sense to me so I can go back to review if
needed - then click <save>. This action renames the file and closes the original _blank
version - ready to be used on the next report.

I then print out a hard copy to mail into the disciplinary committee.
