Washington State League + Tournament Cup Matches

Send the Washington Youth Soccer Game Roster Sheets to the state office:

State League + Cup Misconduct Reports must be filed online at WASRC website

PDL Misconduct Reports can be filed online at WASRC website or sent to the address at the bottom of the roster sheet
(change the 'Authority' to PDL when using the WASRC Misconduct Report form)

District 5 League matches - use the WASRC website
(change the 'Authority' to WSYSA - District V when using the WASRC Misconduct Report form)
