All Washington State League (WSL), Player Development League (PDL) matches will use the online
report system at the Washington State Referee Committee (WASRC) website

All WYS Cup Tournament matches will use the WASRC website

All District 5 League matches will use the WASRC website
(One exception - if you are a Oregon registered official - use this form - and we will transfer the information to WASRC)

CSRA High School Game Report

CSRA assigned - USSF Adult & Youth Game Report (all center referee's are required to fill this form out for each match)
DO NOT file a CYSF or ECYSA recreational match report on this form
